This is too simplistic: Correlation does not equal causation. I think another correlation was the big increase in paperwork from insurance companies for the doctors to get paid, so the docs hired billing managers and administrators to oversee this paperwork rodeo.

Another correlation might be the rise in expensive imaging devices (MRIs, CAT scans, PET scans, etc) that started about this time.

Expensive imaging devices do not require an increase in administrators. And such a huge disparity cannot be overlooked as simply being related to paperwork. This seems like a perfect time to call Occam's Razor into play.
